Immunizations are available for children and adults. Please read eligibility and cost information below. Vaccine Records are available upon request.

Children's Immunizations

Ages: Birth through 18

Most health insurance plans are accepted. Free VFC vaccines are given if you:

  1. have NO health insurance
  2. have Medicaid
  3. are an Alaskan or Native American
  4. have health insurance that does not cover specific vaccines such as HPV or Hepatitis A

Adult Immunizations

Ages: 19 and older

Please contact the Health Educator at 765-764-5855 or HealthEducator@warrencounty.in.gov to schedule an appointment. Payment by cash, check, or money order is accepted and due at the time of service.

  • Hepatitis B ………. $65.00/dose
  • TB Skin Test …….. $35.00
  • Tdap ………………… $60.00/dose
  • Varicella …………… $140.00/dose
